Thank you for your interest in serving on an international mission team this summer!  We look forward to serving with you!

If you are participating in a 2011 Foreign Mission trip, please fill out the following forms as soon as possible.  Once completed, save the file to your computer and attach it to an e-mail to keithf@aumc.net as well as to the trip's team leader.  We will NOT be able to place you on a team until ALL forms are submitted* and a $100 trip deposit is received by the Finance Office with your name and trip clearly marked in the memo line.  You may also make the deposit and any future trip payments online here.  Teams are filled on a "first come, first served" basis of accepted applications.  Delays in returning these forms may jeopardize your participation. 

*If you have participated in an international mission with AUMC in the past four years and we have your forms on file electronically, then you need only to complete a new Missioner Profile and submit your deposit (unless you have had major changes in your health, in which case we will need you to submit all new updated forms).
